Free in-home assessment
- We will support you to ensure the TDHB, GP and ‘In Home Assessment’ is stress free. We personally assist to make the process easy for you with our guidance and support. There is no cost and obligation to receive the free assessment.
- The assessors will come to you regardless of where you are - in your home, in the hospital or in a residential care facility.
Care & Wellness Plan Development
Following the assessment, a case manager will; in conjunction with our prospective residents and their families, develop a care and wellness plan individually tailored and responsive to specifically identified needs, problems and goals.
Service Delivery
The services you require will be delivered on the days and at the times most convenient to you.
24 Hour Support
All residents benefit from our 24-hour support service, which allows for peace of mind should unexpected needs arise.
